Consider this: When developers are trying to set the difficulty of any game,
they have to look at the target market and ask themselves what the players would have the most fun with.

Quake Live is an excellent example of success in this regard.
F-Zero GX is a decent example of failure. Though personally I loved that game, most considered it to be too difficult to be any fun.
Another example of failure would be Diablo II: Lord of Destruction. The game is simply far too easy.

Also, consider this: The ladder season resets every six months to foster competition.
Competition on the ladder is not possible when every top player is level 99.
Log into Battle.net and take a look at the classic ladder. Do you understand now?
