well i saw few occurances as well

1: in a pub pvp game, someone had messed up the npcs (was the bridge map in act 1) so that akara was out in the blood moor and most of the other npcs were on the bridge.

2: also in a pub pvp game, a barbarian had done something, not sure if hack or messed something up, but he was litterally unkillable and he was just standing in one place.

3: was in a cow game, where we had actually cows on our side (not revives or anything) but i think the game bugged/lagged after we spammed mindblast on them so even after the convertion went off, their were still attacking other cows.

these is a few i can remember from the top of my head, ill be sure to post more when i remember them.
