ladder is a number of different skills wrapped into 1:

how to play the game fresh
how to do a rush with no gear
how to use the most minimal of items yet still get the results of high level items - it just takes more time
lol getting though hell with a shit craft caster amulet + any base spirit sword (cs) + lore helm + any shield and a merc with insight on a low damage base doing like 100-200 damage yet still killing hell baal - solo

perks:
getting to level 99 fast
getting an enigma or infinity fast fast fast
early ladder trades high profit
