No, 4 socket items can't drop in normal, it's capped at 3, 4 in nm and 6 in hell. You could cube or Larzuk the base item though if they are the right ilvl.

Nightmare Countess can't drop Lum from her rune drop table. You could run her for 3 Io Runes + 1 Flawed Topaz to cube to Lum.
