As of the latest patch (2.6), you can upgrade ANY item, whether it be set, unique, rare on both Ladder or Non-Ladder.

The recipe to upgrade would be Pul, Ko, and a Perfect Amethyst in a cube. Bear in mind the base defense will roll, and will be anywhere between 50-60.

Yes you can do that as stated before above .

1 Ort Rune + 1 Amn Rune + 1 Perfect Sapphire + Normal Rare Weapon = Exceptional Rare Weapon
1 Fal Rune + 1 Um Rune + 1 Perfect Sapphire + Exceptional Rare Weapon = Elite Rare Weapon

1 Ral Rune + 1 Thul Rune + 1 Perfect Amethyst + Normal (Basic) Rare Armor = Exceptional Rare Armor
1 Ko Rune + 1 Pul Rune + 1 Perfect Amethyst + Exceptional Rare Armor = Elite Rare Armor

rare armor = all armor types , so gloves/belt/boots/armor/helmet/circlets/pelts/shield
