How Does % Health Steal Work? - Topic

So the feral berserker set at 6 pieces gives 40% life steal, so is that 40% of my damage output to health? Or is it % of my max hp each hit? Or it could be % of enemies health...that would be cool

I would imagine its of the damage actually done to the monster per hit.
