I can't click on any items pickups or npcs. Anyone know how to fix this?

Full inventory and what npcs?

this problem is in the beginning of the game i have one item in my inventory and its the npcs at the start of the game

Not sure what the bug is, I experience it sometimes as well...namely when moving items from my stash to inventory.

The only fix I have is left clicking the item first, then you can shift click it to move it automatically....

Just restart the game also fixes it.

if restart doesn't fix send message to torchlight support group,..
