Found this bad boy in act 4 NG+

May i ask what NG+ is ?

It's essentially the nightmare of TL2, the difficulties go NG, NG+, NG++, NG+++, NG++++.

Not very creative names, but it gets the point across.

Is it a mode you unlock or something you have to install?

You unlock them after killing the final boss in A4, but be cautious when going INTO these new difficulties, once you are in a difficulty you CANNOT go to the lower difficulties.

Oh wow, i'm only on veteran so basically just work your way up until you find OP gear?

Veteran is a complete different difficulty scale. You can change the Casual, Normal, Veteran, Elite difficulty at any time, this more affects the LEVEL of the zones you are playing in.

So it's really a BIG jump is what your saying?

Well it's not necessarily a jump in difficulty, I guess that's the wrong wording, it's more of a jump of Zone levels, for instance Act 1 Zone 1 is only level 1. Whereas in NG+ Act 1 Zone 1 is level 51.
