Was watching Toronto FC last night , Nick Haglund took out a player in the 88th minute , Red could have been the call or yellow, the Ref pulled out and showed Yellow, then about 10-15 seconds later after players were yelling at him, he went back and showed a red card...

once a call is made is it not final?

He can discuss it with the other referees and go back on it, hardly ever happens tho.

Yellow for foul, yellow for dissent. Yellow + Yellow = red.

Have you ever seen it before?

Looked bizarre and all the Toronto FC players were confused...

He changed his mind from yellow to red...perhaps on word of the assistant

Happens a few time every season, 99% of the time its because the assistant says so.
It can also go the other way around, if the ref wants to give red but the assistant says it wasn't a red etc, it's all about the communication between the ref and the linesmen/4th official.
