Italy is actually better equipped than the US to handle an outbreak in terms of hospital beds per capita. If we have an outbreak even remotely like they have had, it is going to be really really bad.
They are literally turning people away and refusing to serve people who are older and/or have co-morbidities, who have less of a chance of survival, and are focusing their efforts on the severe cases of younger/healthier people.

This is the danger of COVID, the delayed manifestation of symptoms/mild cases cause people to walk around spreading the disease before they even realize they have it.
