in japan you can pretty much always get to a big city by train in an hour or less. In Tokyo there is a ton of English, and people are not going to try to speak Japanese to you. even if you speak to them in Japanese, they almost always will try to speak to you in English, or just go get someone else who knows some English.
smaller town will make you have to use Japanese more, and that will force you to learn it faster.
I have lived in japan for some years, and studied in both Tokyo and smaller cities. Smaller cities were much better in my opinion. you can live in Tokyo easily with 0 Japanese knowledge, but living in the countryside will make you have to use Japanese.
also, Tokyo gets pretty old after a month or two, Japan's nature is much better imo.
