Danzo prob got it transplanted. He probly heard Kakashi did it and was like wtf hax im do that also and just recruited one into his root and took it or jumped one on the battle field and claimed he was KIA. Friendly fire is pretty common on the field

but imo Sasuke will kill him or Madra cause his body isnt built for it and it makes him weak pretty quick

btw isnt kinda weird that kakuza(fixed) defeated the 1st hokage and stole his heart but the all might madara loss to him
so that means naruto can kill madara in a second since he owned hidan with the rasen shuriken
