Kotonoha was just so psycho that she thought there'd be a full grown baby in there after the short amount of time. Plus she stabbed her in the gut, not right in the cooch at a perfect angle to see a fetus...

Plus the statement was more of a symbolic (for lack of a better word since I just woke up) statement about Sekai's character as a whole imo.
