There are a lot of people who never got what they desired in this story. I don't think Dan saying that he wants Tsunade to live a long life has any significance really. If you could see your loved one as a spirit one last time, would you say that you wanted them dead? Besides, Dan's presence is far less significant than Naruto becoming Hokage, and I doubt he's going to become Hokage at this point in time unless Tsunade bites the dust, which leaves a few options.

1. Tsunade lives, Naruto doesn't become Hokage until she dies (he likely pulls a Jiraiya and travels a lot in this time period, as some expect) some 20 years down the road.
2. Tsunade dies at the hands of Madara, Naruto is named the new Hokage.
3. Tsunade lives and steps down as Hokage, thus giving the title to Naruto.

Those options all assume that Naruto IS going to become Hokage. I can't see option 3 happening at all. You never know, Naruto could refuse (which I doubt) if he's offered the title in favor of traveling the world or something as well.
