No it was in a flashback i believe while she was running she had it on her waist. Maybe when fighting soifon i am not sure.

I know i always make mention of this but still its just to blatant, she fought to save the world(s) basically and didnt mention it or carry it so either its like shunsui's and isnt suited for teams or like unohana/ikkaku/5th seat 11th she has a vow on why she doesnt use it.

I think i asked this before but i dont remember if it was answered or not but hisagi he has a bankai and just doesnt use it due to that fear shit or was it a translation error and that vizard/captain with the combat knife is teaching him it? which btw is dumb since it can be taken away LOL lulz bleach

Perhaps the shinigami will now prepare for their bankai to be stolen and have a counter measure for it? I dunno, I just think that aspect of the story has already made its impact and Kubo will add a new element to it that alters the battle. If more shinigami are running around getting their bankais stolen, I'm just going to facepalm.

I think Yoruichi simply doesn't use a sword because it's not her fighting style.
