The entire time (from the second episode on) he makes errors because of his own arrogance. That along with the errors of EVERYONE else he ever associated with were always his problem. His arrogance grew and grew. After episode 26 he started really believing that he was an infallible God and he acted accordingly to that notion. I love seeing his slow decent from methodically precise to chaotic and misguided. It's an entire show that chronicles a brilliant man's decent into madness. I know I'm a fanboy, but there's plenty of good in the late episode of the show and after I had really thought about the ending, I came to the conclusion that there was no other way to really end it.
Edit - The only thing that really bothered me was the hotel room meetings and how sloppy he was there. Once again though, I think that's just another show of his arrogance. He felt the rest of the NPA were so insignificant (they were) that he didn't have to do a damn thing about it. All of that ended up being pointless. Light had made it so painstakingly obvious of his true identity to anyone not associated with the NPA that he didn't really have a lot of options other than a pure offensive approach.
^Came to argue but it's a rather good point, still he was so clever/mythodical like you said, went a bit mad but shouldn't have been so arrogant when things started to go wrong/how much danger
Need to rewatch think it was like the first anime i watched all those years ago
It's still super good despite my opinions on the end
