Yes, but the way it was shown is most likely a tease, they show her being wheeled into surgery and then him standing over the grave. Too much reveal for just a preview clip to be the actual plot.

Since I did not watch the preview, and placed my thoughts on the show, I did not see the wheeled into surgery part. Thanks for sharing...
If she is not dead, who in Olivers circle know? Does he even know, some group could have arranged a fake death but not letting them know?
There are many scenarios in her being DEAD... they know, they don't, or she is dead and someone clones her DNA to infiltrate Arrows group.

There was too much reveal... which gives an indication that it is a fake death, but, if it is real, it is a well played preview.
