Where was he coming from that he needed to be picked up at the train station? I assumed he was coming back to visit after having moved to New Mexico.
I'm having trouble seeing this: He hops buses/trains across the country, lays low in NM for few days, then hop buses/trains back all without needing his gunshot tended to. Then, the instant he comes back to the town where everyone knows him, he suddenly needs to ask a stranger cab driver for a secret spot where he can get his wound treated without anyone knowing.
he took the train once which is from philly to new mexico to meet his daughter in law after killing the cops
he stays in new mexico, not laying low, for his daughter in law and grand daughter, his first time in this town so he asks cab driver for special doctor
u've mistaken the flashback scene of him killing the cops (which happened months ago) with the current time line

mike gets picked up by detectives, calls saul, goes to see daughter in law asking if she talked to the cops, leaves angry, comes back, explains everything
flashback:
mike kills cops in philly, takes train day after, visits daughter in law with maxipad on his shoulder, calls cab -> doctor
