Only lucifer and Michael are left, but are trapped.... everyone else is dead.

how did sam get demon blood in him again?

First time, Azazel making his special children to find Lucifer's future vessel, when sam was a baby.
After that, ruby was helping him get strong enough to take on Lilith and be able to accept Lucifer's power.
After the one battle where he showed up on the plane he was essentially purified but still had the mental addiction.
Season 5 he drank only a few times, though he massively drank before taking on Lucifer being the last time he drank demon blood. This was most likely the point when he drank so much that it altered him forever.
Though it was a little lazy on the writer's parts to completely remove the demon blood from the picture for season 6 and on given that everything we've seen from it.
Unless of course lucifer and michael beat any cravings out of him while he was in the cage.
