What probably happened was the wrap around his torso started up higher and moved downward during the filming of the episode.
They stained the correct location of the wound and wrapped it correctly,
but during the shooting it started to move downwards and they didn't think to correct it.
If you notice the bulging at the bottom, that's a sign that it has been sliding down during movement.
