People die when the bite wound gets infected. Cutting of the limb prevents the infection from getting worse because it's easier to look after an amputation wound than a bite wound.

Everyone is already infected. Bites and scratches do not infect you, they cause you to have a severe fever and die from it.
Then you reanimate as a zombie. Think of it as a sever case of sepsis, sepsis doesn't spread immediately.
Removing the limb moments after being bit or scratched prevents the fever inducing infection from spreading, it doesn't always work.
Dale is an example of it working. In the comic, he had his leg cut off after being bit. He survived only to later get bit in the shoulder and get kidnapped by cannibals who eat his other leg.

yea, the writers check jsp, and virgin forums to see what the nerds are saying, poor viewing figures n all that..
nothing to do with the natural progression of the story..
none of the episodes in season 2 had gore/action non stop, and people dying..

A few people on jsp definitely represent the 9 million people that watched season 2.
