I assume word travels the grape vine about most things, perhaps the winchester dont get credited with everything they do, but afterall they are just humans - and monsters are usually arrogant and believe they are stronger. But there's also the side of the coin that the boys are 'hunters' so even if the monsters are to run, they will be 'hunted' hence left with the option to fight. I mean, the boys are usually hot on the trail because the monsters etc need to feed/do something that
does leave a trail.

I think the torture vision was Sam remembering the cage for some reason, the show beginning the same way as swan song with chuck/god really hints at God maybe doing something this season...
