Really depends on hand size and your type of grip.
These are the best mices from what I tested. Never been a fan of any corsair, asus and razer mice models yet.

Ideal would be to try them out in store. My money would be on the G900 chaos. Currently using the g502 and I luv it.
As for mousepad you fist need to messure your ideal size and if you wanna it under your keyboard as well. I've been using steelseries qck, roccat taito and Coolermaster swift-rx. I can highly recommend a mousepad with stitched edges!

That is subjective. Left handed or right handed?

For a mouse pad Steelseries 9HD or Mionix Propus 380 worked for me. Now I find I want something bigger. The 9HD is my current, 12x10 inches. I lost my Propus 380 but it was 14x10 inches. I find myself falling off my mousepad sometimes. Maybe 15x15 or something.

Like I said all subjective. For mice I've used Steelseries Sensei and Mionix Avior 7000. Those both work great for me. Anything else I've tried is trash. Mice with weights and all that are fun but a quality sensor is better. Those two mice are ambidextrous so they work for us lefties. Never seen a left handed gaming mouse that was worth a damn. Also fuck razor mice. Those sucked for me too. Also any off brand mice usually suck. Best to fork out the cash for a decent 5 year investment.

200 bucks is a great limit.
