Its too old, the motherboard, but also the other parts. like you could maybe upgrade the RAM(which btw I didnt even see that in your specs) but it wouldnt change much. Your GPU is like 10 years old. You need new everything.

PS: you could possibly get an SSD instead of the HDD, and add more RAM, thats all you could do imo with that setup, but I dont know how much RAM its got, or the HDD.

When I run the task manager it shows my CPU runs at 12% to 25%. So, I don't think it's my CPU. However, the memory category and power usage are very high at 76% to 96% when running Path of the Exile. Any ideas on how I can fix this?

you would be better off playing path of exile on a xbox with keyboard and mouse

I don't know what that processor is exactly either. But I think it's safe to assume it's an Intel i3 4th generation, in which case, I have no upgrade advice for that.

As others mentioned, wasting money to upgrade outdated stuff. Best to save money and buy new
