update drivers
try running cmd prompt admin, type "sfc -scannow"
try running tweaking windows repair
do you have task manager open when this is happening? how much ram? if you have performance tab open you can see if something is hitting max usage.
may want to also run something like malwarebytes to make sure there are no crypto miners on your pc

Its all the Blizzard games & path of exile.
Do to the new Direct X 12
Simply what I do to fix this issue is turn Vsync off, and then set my Target FPS to 30 , and cap FPS to 60-75-120-145-165-240 depending on monitor / settings / game

Feel free to add me on discord if that doesn't work out for you. Might be able to find solution for ya.

Instead of telling everyone to nuke their fps you could also just suggest to use dx11 if that's really the issue?

tbh the last time i had random fps drops i had to redo my entire windows lol
if you ran tweaking windows repair and the sfc -scannow then i'd try that out for a while
