yeah intel hd 4600 can be iffy sometimes in PoE, especially in certain boss fights with a lot of assets on screen. However where it really might become an issue is if you only have 1 stick of RAM, the cpu and igpu sharing a single channel in a game like PoE sounds like a bad time.
also, your os is probably a dumpster fire judging by how much you know about your own hardware specs, can only imagine how many pando media boosters, malware, control panels, virus scanners, toolbars, driver doctors, etc. are running in the bg at all times.
In any case you should get a gpu. For that cpu a 1060 would be good in general but if the most demanding game you are playing is PoE i would shoot for a 750 ti or 1050 (non ti), either would be cheap, fast enough for PoE, and probably enough for your PSU to handle as no external power is required.
The only benefit for springing for something like a 1060/1070 or even a 1050 ti for would be for higher triple digit numbers in the hideout. When fps truly matters in PoE i.e in maps, breaches, intense situations where you might actually die end-game, gpu usage goes down significantly as the cpu does most of the legwork. It's not a very demanding game in the GPU department in my experience.
