If you want to do it yourself, take the root note of any chord being played (i.e. the A in the chord A),
Match that single note on the guitar,and build the right chord pattern for that note.
Then shape them with the rhythm of the piano version
Considering you don't have to transpose note names, it shouldn't be too hard if you set yourself to it.
Music is more about progressions of patterns, the names of chords are just a tool to communicate them.
