Idk, but I think if you claim, it is transferred into stash and b/c of poor stash-management, people dont find those items anymore and think it "disappeared"

When you farmed blood harvest, if you didn't collect rare adn legendary items and tp to town it will disappear.
To advoid it:
1- Collect and drop items to the ground, it won't disappear when u come back
2- If you have a team mate, no need to collect and drop, tell him to stay when you tp to town, when come back it won't disappear
