Salvaging legendary gear has a chance to give forgotten souls I think... other than that it's the screaming hell veins and chests in helltides

Screaming hell Vains drop them. The bosses that falll out of the sky and show up on the map during a helltide and the helltide chests. These are the 3 places I get them and yea they are all from helltide

Ngl I've salvaged over 100 ancestral legendary items and only got 5 or 6
