I justed looked a bit through the internet and youtube to see what is possible. So, it looks like the Living Steel drop out of helltide chest on WT4 is always three.
But there seems to be a chance that single Living steels can drop independent of those three we always get. The chance is probably way lower then 2 - 4% though.
In fact its so low that not even a single youtube recording exists (or I simply didnt find it).

Some guy says he got 4 Steel once and 5 Steel once in a span of 750(!) helltide chest openings. So I dont know what the truth is and if it really can happen, but its fair to say, we probably get three almost all the time.
