The number of sockets is only affected by the ilvl of the item, which is based on the mlvl of the monster that dropped it.
If you got your white crystal sword in A5 normal than the ilvl is most likely between 24 - 43. Some mods/tools are able to show the exact ilvl of your item if you are not sure
For crystal sword the following sockets will be applied for the following ilvl
3 sockets = Ilvl 1-25
4 sockets = Ilvl 26-40
6 sockets = Ilvl 41+
You can read more about this here:
https://diablo2.diablowiki.net/SocketsTL;DR
If your item did not drop right at the beginning OR the end of A5, you will most likely get your 4 sockets
If you want to be 100% sure, determine the exact ilvl of your crystal sword
Well, thats not totally correct.
There is no monster in act 5 with lower mlvl than 26 so every crystal sword dropped by monsters will at least get 4 sockets (chipped gem cube receipe not considered).
The only way a crystal sword will get 3 sockets is if it is dropped by chests or racks in foothills and highlands (alvl24 and 25 respectively).
Also there are mobs in middle act 5 levels that have mlvl of 41+ so a cs dropped by them will get you 6 sockets. On the contrary there are mobs in WSK3 (alvl43) who have a mlvl of <40 and therefore you will still get 4 sockets on your cs if dropped by them. If it is dropped by a chest in WSK3 it will get 6 sockets.

Just curious, why would you use the socket quest? Why not use the cube recipe?
To ensure you'll get the maximum number of sockets possible
