When you upgrade an item from normal to exceptional it receives a hidden level requirement tag of +5
When you upgrade an item from exceptional to elite it receives a hidden level requirement tag of +7
When you upgrade an item from normal to elite it receives a hidden level requirement tag of +12

The level requirement tag is added to either: the base item level req, affix or socket holder, which ever one gives the item the highest level requirement.

Your item upgraded (Martel Del Fer) would have a base item level req of 25, highest affix level req of 18 and no socket holder, so 25 + 5 = 30.
