Most probably intended design, otherwise melee range characters would be at a serious disadvantage in Nightmare and Hell where the lightning inferno is way more potent.
The second image on the following page displays the safe ''circular disk(s)'' for Nightmare and Hell difficulties:

That Assault Helmet has triple durability and is thus a failed Assault Helmet (none exists).
I would sell it; Tarnhelm is simply better: +1 to All Skills applies +8 to e.g. Axe Mastery +% Attack Rating, so already there it beats the 5% Bonus to Attack Rating modifier.
Yeah, not far into A5 I tossed it on and off to compare stats in the character tab. Didn't take long for hindsight to kick in. I soon after found a circlet that boasts enhanced damage, dexterity, and a much greater additive to attack rating, and even it doesn't really hold up to +skills. Axe Mastery and Frenzy for ar and dmg (also Double Swing if I should happen to use it), Natural Resists, Iron Skin, all the warcries, Berserk in the future. Hell, even leaping further is nice, and of course a nice chunk of mf. I haven't played a barb thoroughly in quite some time.
Also thanks for reminding me about the triple durability rolls. It's always nice to check and see if an item you want the unique version of has this, so you can come to the realization that it would have dropped had an enemy of the appropriate level dropped it, and then stew on it for a while.

I believe 4 set Slayer Guards dropped before Arreat's on the last barb I played. But, these weren't failed uniques, that was just me being unlucky. I digress.
Thanks for staying a while and having a read.

I always appreciate it.
