Has anyone seen this before??? happens when i try to go to act 2 (ive killed andy twice via quest kill)

hmm... is pretty strange... that is just common error but shouldnt happen from changing acts

so it says my quest is done but every time i try to go to act 2 it goes back to the error code and shuts my d2 down...
