In PvM you don't need extra defense from upg. You are not duelling barb.

Swap the ber runes for 15ias jewel/#@
Make sure grief is 35ias +
Swap gore for goblin toes
Swap dungo for SoE
Swap 1raven for Dwarf

Upped Hoz also gives a bit more smite dam
With the 2 x 15ias jwls you'll be able to smite at max frame with salvation

I prefer Fanaticism not only for IAS, but also for ED. That way you can use Ber runes because Grief at 30 IAS is enough if PB.
I said why prefer Gore in previouse post.
Dwarf or Wisp is good instead of 2nd Raven but I personaly prefer 20 dex.
Verdungo much better than SoE. Why swap? Both 15% DR but Verdungo also have 30-40 vit and 10% FHR. The MDR from String is not %. Also life leach do not work with Smite.
70% of the smite dmg comes from Grief, that's why using non upgr Zaka is better in my opinion.

Fade pre-cast is only for Uber Tristram and can be replaced with CoH (instead of Forty).
