How do you get it? Only from divi cards? Chancing white leather belts?

u can obtain in a lot of ways.

as drop or chance on a nemesis zana mod map.
rerolling unique belts with ancient orb (on a nemesis zana mod map)
fiend or the doctor divination cards gives directly the item
also there are few other div cards that have headhunter belt as outcome
fractured walls in delve can drop headhunter too
next league u can get from alva's temple and best recipes too

Shaper's fortress maps can drop league specific items aswell.
