No. Neither of those. It must be Zana's Nemesis Mod from the Map Device itself. Any other way will NOT WORK.

Depends how Bestiary & Temple system got changed, or not, but both can also provide it with unique belt recipe (including league unique items)
Have to wait the confirmation from GGG / in game.
