I've learned to build mostly around defense or a leech of some sort, regen, ect. and leave the damage mostly to weapon and rings and an amulet.
Be patient too when reaching for far passives and know you can always create quick routes in the node trees and rid of them later when connecting from further epic nodes.

after killing vaal on cruel I have clicked on the exit from the room
unfortunately I was not moved to the act 3 but to the login screen
after login I noticed that my char is already in standard league

yah that simular thing happened to me with a dc and no monsters around and when I logged in and started rummaging through my stash notice somehting different, lol.

Died when I used a mana potion that takes 15% from life while I was CI. Was instant and said I committed suicide LOL

By the time I was loaded into the map I was at the revive screen.....
