I spend a total of 500 alterations on HCSSF to try get my Quicksilver flask with Adrenaline. I know RNG is RNG so I just kept going assuming that mod is very rare and I just had bad luck.

Now I found out because I got the flask from Quest reward it got nerfed and its not possible to roll adrenaline on it. How is a new player supposed to know this?

Are there any simular ninja nerfs I should know about?

I dont think its a nerf bro.
If it was the flask you from bestel in act 1 the ilvl of the item is most likely the problem. You can use something like poedb.tw to find out what mods can be rolled on what items, and what requirements they have te be rolled.

Its item level mate, low level items cant roll all mods.

The adrenaline affix is locked to ilvl5+ so just look for that.

What about chemist mod?

the 3.6 rares, somehow nerved all existing items, and destroyed hybrid defender builds.
