Usually 'hell found' magic amulets and rings are good enough. But if you'd really want to make sure you're getting top ilvl drops. You can farm them from hell baals.

Yes, gambling will allow you for the easiest high level jewelry. As to who specifically you gamble from, that doesn't matter

with a lvl 90+ character but lvl 94+ is ideal
