not impossible, there are some screenshot file exploits out there which can download and install a .exe by simply opening the image (most of them are patched, so make sure you have your windows updated)
not sure about notepad tho

the site he downloaded it from can also download a keylogger without him knowing (assuming its a random / non-populated site) or just a pop-up which redirected to a browser exploit (drive-by download)

anyways, i highly doubt it, hes probably lying or downloaded something else

No, my friend got keylogged and i've sent him a notepad file + screenshot of d2 item, and thinks it was me, so just trying to get some proof it wasn't.

It is possible. You can create a file that looks like a notepad, once double-clicked it will trigger the keylogger to open in the background, as well as opening the notepad document.

Changing Icons to deceive is very easy Not many people enable Show File Extensions.

When you download it, it says what it is.. windows will warn you before you open an exe
