Yes it is possible. You'll have to change it in options in-game.

Always the same, copy and paste and I think it is allowed to ask d2 related question in general help afaik.

I know i can change the hotkeys ingame but i want to create my own hotkey with my mouse+keyboard.

Changing them in game & setting them to whatever keys u want to use is the prper way to adjust them to ur liking

You can't use any kind of .txt or .ini to change your hotkeys so just do it the simple way and do it in-game.

I am not talking about 3rd party "mdifidificaton". E.g Right mouse click = Holy Shield

The only possibility is to change it ingame, anything else might get you in trouble with Blizzard.
