No one knows what the Black Mushroom is for, My guess it's for a secret portal. In example like wirt's leg for the Cow level in diablo 2.

black mushroom was also a quest in diablo 1.

Wirts leg + Town portal scroll = a secret cow level portal.

Im guessing this has the same effect on D3 with useing the Black Mushroom.
