Only skeleton king drops.
Basically you run to him as fast as possible, kill, stash drops and your gear, make new character and start over.

SK dropped this one.

It's false that only SK drops it; I've had friends find all kinds of rares from floor holes in Defiled Crypts and Cathedrals.

Virgin SK is always a drop though.
