Anyone know what the heck is happening with this?

I sharded Act 1 and got 3 heralds before catacombs. I drew ire of a herald in Catacombs and then finished A1 no herald. I popped an act 2 shard and cleared every zone and no herald spawn still. ????

Yep, it's not rare to get 0-1 heralds after ethnic cleansing a whole act, no children and women spared.
