Players that were banned were removed from the ladder.
On HC, a 98 and 97 that I sometimes got into games with had their chars removed from the leaderboards. Can confirm they were banned because I've seen them say it in a discord channel, as well.

As some who has played both SC and HC over the decades, I've found HC players are usually more kitted out with programs.
Which is why I always get annoyed by people saying to others, "oh. You play SC? That's so lame. You should play hc. It's so much harder and challenging."
1) the typical HC player is usually afraid to do "harder" content. *yawn*
2) most on HC are running chicken, tmc, mh, etc.
3) as an experienced player (have made a few 100% legit 99s and 98s in the past...I know pvm quite well), HC isn't harder. Not dying is pretty damn easy to do. Just don't play like a nub. The biggest chance of dying is server/connection based issues.

I think even game timers that the speedrunners use are considered third party software. Luckily they play single player offline and thus not at risk for cheating.
There are 2-8 man speedruns as well on live servers, too, including by guys that are Blizzard partners.
However, their timers don't interact with D2 and are just screen overlays. For the timer to mark something, they manually enter a key for their timer to mark the time (so each time you hit, say F12, for example, it'll mark a time). Speedruns are then officially reviewed by the speedrun community for the exact finishing time.
