if you forget to temp-perm and save against the server what I just said will happen, i.e. the latter of the duplicated item-ID will poof (however the first to save will not).

Some oddities (chobot debacle in specific) have appeared but we're yet to see substantial proof of duping on any scale beyond either 1) irreproducible occurance 2) private one which for all we know could've been in the game for a year already.
This does not equal confirmed.
It's entirely possible something like this is occuring, e.g. as a result of blizzard trying to fix the memory overload bug (poofing crafted items)
