Concentration archers with amplify damage and cursed affix. If teleportation on top, have fun. H5's minions also hit hard. Extra fast ghosts with concentration can also fuck you up.

I have died to 2 of em so far was claw viper fanat in ancients way or some a5 cold cave and one you forgot to put : the undeads with big swords who also charge, the ones who wake back up, same ones as pindle.
I guess fanat + amp + high phys hurts
