I recently purchased a car from Kentucky and I'm trying to figure out what I need to fill out to make it my car in Tennessee.

^Example of Kentucky title
I'm not entirely sure what I need to fill out.
I have his signature and printed name, notarized with the date and the notary's signature.

I don't remember every buying something from KY but, any other state i've purchased things from you simply fill out the buyers section (usually printed and signed name with a date) and then take it into your local DMV and they handle the rest. The will the send you a TN title in your name.

I've just never had to take an out of state Title into the County Clerk, let alone have a title that had the Notary on it.
To my knowledge, I need to sign it and put in the odometer. Anything past that I'm not entirely sure what all else is needed of me.
A lot of the crap looks like it should be filled out by me, but I've never had fill out anything more.

The owner should have filled out:

Print & Sign sellers name
Selling price
Odometer (if required - Every state has their own random rule, 10 years or older and it doesn't have to be filled etc.)
Date

The only part you need to fill out is:
Print & Sign buyers name
Date

From that point you take it to the Department of motor vehicles (or your states equivalent -- From what it looks like on google y'all call it vehicle services division)
Turn it into them with your states title transfer fee (usually 25-50 bucks) and they will then convert the title into your name and give you a TN title in the mail. During the processing time they will give you a printed sheet for temporary registration.

All of that other crap on the title is for dealers, lease agencies etc. You did not trade in a vehicle and you bought it from a private party. There is nothing more you should need to fill out. (all of the information on titles is for sales tax for the most part. and since you went with a private party the sales tax is simply based on the selling price of the vehicle.)

I know the entire front side I pretty much ignore.
I've written and signed my name to the right of the yellow portion of the sample title.
I've also written in the odometer in for what was written on the receipt that both he and I had signed.
As far as the amount paid goes, I'm trying to figure out where it would go.
I see below the "Application for Title/Affidavit of total Consideration" it shows "sale price" but that shouldn't be it.

So it looks like KY doesn't have a "sale price" on their titles. They make you have a notarized B.O.S. along with the title.

With that being said. Sign and date your part of the title under "Buyer" (fill in your address if applicable -- some states request it, some dont)

Take the title and B.O.S. into your Vehicle Services Division in TN. and they should be able to handle the rest. Honestly, if you're not out in the boondocks and somewhat close to a VSD, just take the paperwork to them and ask them what else you need. Take two forms of identity as well as a check book or credit card to pay the sales tax and title transfer fee.

I'm sure the others can chime in if they feel you need anything else along with you but that's all I would assume you need.

If you still feel like you are missing something, just take it down to them and ask them what you need to fill out. They can walk your through everything as well. Don't fret. They are always pissed off people and live a miserable life. It's not your fault.

Thank you very much, I just wanted to make sure there wasn't any strange portion i had to fill out because the title was from out of state.
I ended up having to write down my address on the very top line and put the sold amount next to the sold I had to put the amount I paid.
Luckily the woman there wasn't in too bad of a mood and it went quite quickly.
Now I've just got to wait until it gets warmer out then I can switch my insurance over and put her on the road.
