Now I've only bothered taking one toon through Hell yet on the Test realm, but here's a couple of drops I didn't expet:

NM Forge - Um followed by
Hell Forge - Gul

Hmm, Tell me this is just an outrageous fluke and not a new setting to drop top rune every time?

Duriel dropped 2 x Essence on the quest drop Andy dropped 1. However, they are not in any way guaranteed as far as I can tell. Meph didn't drop his at all.

It will be.....different, not sure how yet, atm it is running simply the new beta patch, I am testing it out atm and will need some test dummies later

Setting this up is rather difficult though.

Still (naïvely) hoping they will eventually modify it to drop from {r23, ..., r33}, either uniformly or using reasonable frequencies.
